A representative of the city's medical institutions will be working in the Centre for the Delivery of Administrative Services for completion the declarations with a doctor. That decision was taken by the deputies of the city council during the regular session.
Now it is not necessary to go to the hospital to sign the declaration with a family doctor. Clients of the Centre for the Delivery of Administrative Services can come to receive the necessary administrative services and “kill two birds with one stone”, having signed such an important document through the medical representative. It is a good practice to choose a family doctor in advance.
We recall that in order to sign the declaration with the doctor with you must have a passport (for a child under 14 years – a birth certificate), identification code and mobile phone, which will get an SMS with a code to confirm registration in the system.
